The enquiry form is configured for Netlify Forms. When deployed to Netlify and submissions are enabled, information submitted will be available to the site owner through their Netlify account. Only include information needed to respond to your enquiry. Do not submit patient details, health information or other sensitive data.

Participant portal

The participant portal on this site is a static demonstration. It does not authenticate users, save log entries, process electronic signatures, or store or transmit uploaded files. Do not enter real credentials or personal information.

Before launch

This starter site does not replace a reviewed privacy policy, participant terms, travel terms or security assessment. Add the organisation’s approved contact details and policies before collecting enquiries or inviting participants.
